Social Worker – Care and Community Our client, Ais looking for Social Worker – Care and Community for the following Provinces: Gauteng, Cape Town and KwaZulu NatalREQUIREMENTS:The ideal candidate must have:o A relevant degree in Social Worko A registration with the SACSSP (South African Council for Social Service Professions)o At least two years experience in the Non Profit Sectoro A valid driver’s licence and own caro Computer literacy and must be able to travelROLE OUTLINECare, Management and controlo Conduct home visits to affected members and their families on a monthly basis as per the target seto Provide psychosocial support to persons affected by Muscular Dystrophyo Facilitate referrals to the relevant medical institutions and professionalso Provide recommendations of mechanical aids for affected persons o Facilitate Support groups for affected persons and their familieso Open, monitor, evaluate and report on caseso Facilitate other support services for affected persons and their families as required by the foundation.o Facilitate care management workshops / forumso Provide reports, attend meetings as requiredCommunity o Initiate Capacity building programmes for the communityo Compile community profiles for the assigned areaso Establish the relevant community networks and partnerso Volunteer management: recruit, establish, train and maintain a patient care volunteer base in order to offer a holistic home based care service to patients and their familieso Recruit, establish, train and maintain health promotion, education and awareness volunteers to ensure that all MDF campaigns are implemented according to the national standards and business plan within the community as per the set targets.o Ensure that programmes are carried out in the relevant facets of the community as requiredo Facilitate educational forums with other health professionalso Develop effective community mobilisation strategies to increase community involvement.Legislativeo Ensure compliance with the Department of Social Development and other service agencies to best serve the needs of affected persons.o Be abreast of professional developments and current information in social work and muscular dystrophy and adhere to the code of professional ethicsSKILLSo Strong organisational skillso Knowledge and understanding of community and its structures and ability to speak a local languageo Ability to effectively communicate information and ideas in written and verbal formato Ability to train other people and conduct public speakingo A Team player who is able to build relationshipsIf you meet the above requirements please send your CV and motivational letter to***email*** >***email*** with the title of the position in the subject line.
